    Our Standard Immunization Schedule Birth to Age 6

Age 2,4,6 mo. - Inactivated Polio (IPV)and H. Flu B (HIB)

Age 3, 5,7 mo - Diphtheria, Tetanus, Acellular Pertusis (DTaP)

Age 12 mo. - Measles, Mumps, Rubella (MMR)

Age 15mo - Varicella (Varivax) _
